Please Fill out the Application form below, and we will have one of our associates contact you. Remember prior to your interview, you will need one essential item, your driver abstract. This document can be downloaded online or can be obtained in-person at your local Department of Motor Vehicles.
To immediately download, save and print your own certified driving record abstract, and also receive a copy by mail, use the MyDMV Get My Driving Record Application (requires sign-in). The fee is $7.00.
You can also pick up your abstract in person at your local DMV for a fee of $10.00.
